This topic includes endocrine medication of medical surgical nursing like antidiuretic hormone desmopressin , thyroid med. levothyroxin sodium, propylthiouracil, vasopressin , dexamethasone , hydrocotisone, oestrogen and insulin . this will provide their uses , side effects etc .

**** Endocrine Medication****
1. Antidiuretic hormones :-
- Desmopressin acetate and vasopressin.
- Antidiuretic hormone are used in Diabetic insipidus.
Side effect of Antidiuretic Hormones :-
- water intoxication, hypertension with Water intoxication, and abdominal cramp, runny nose
- Signs of water intoxication:- drowsiness ,shortness of breathing, headache.
2. Thyroid medications :-
- Levothyroxine sodium is commonly used as thyroid medication to replace thyroid hormone deficiet in condition such as Hypothyroidism and myxedema coma
- Thyroid medication should be given at least four hours apart from multivitamins aluminium hydroxide and magnesium hydroxide because this medication decrease the absorption of thyroid replacement
- Levothyroxine sodium should be taken on empty stomach in the morning at same time each day
3. Antithyroid medication:–
- Thyroid medication includes propylthiouracil, potassium iodide and sodium iodide.
- Medication are used for hyperthyroidism or Graves disease.
Side effects of Antithyroid medication :–
- Iodism :- characterized by vomiting abdominal pain metallic or brassy taste in mouth, sore gums .
- Agranulocytosis with leukopenia and thrombocytopenia.
Intervention:-
- Teach patient not to stop the medication abruptly
- Avoid aspirin and medicine containing Iodine
- Instruct the patient to take medicine with the food to avoid GI disturbance
- Propylthiouracil causes agranulocytosis if patient develops fever or sore throat advise him to contact physician
4. Parathyroid medication :-
- Calcium salt administered with digoxin increases the risk of digoxin toxicity.
- Asses for symptoms of tetany in patient with hypocalcemia.
5. Glucocorticoids:
- Medication include dexamethasone, hydrocortisone , methylprednisolone and betamethasone.
Side effect of glucocorticoids :-
- Hyperglycemia ,growth retardation in children , increased risk of infection , moonface ,obesity, weight gain and hypokalemia
- Glucocorticoid are contraindicated in psychosis and fungal infection.
- Advice patient to eat food high in potassium.
6. Estrogen:-
- Oestrogen are contraindicated in patient with breast cancer, endometrial cancer and suspected pregnancy and lactation
7. Insulin and oral hypoglycemic medication:-
- Metformin side effect most common is diarrhoea and serious is lactic acidosis
- Acarbose should be taken with first bite of food.
- Short acting insulin is regular insulin.
- Intermediate acting insulin is nph.
- **Main areas for injection are abdomen Arms(posterior surface), thigh (anterior surface) and hips.
- Insulin syringe used are 27 to 29 long that is about half inch long
Mixing of insulin:-
- Rapid and short acting insulin are only type of that can administered IV
